What Happens in an Eating Disorder Therapy Session?

If you’re nervous about making contact, you’re in exactly the right place. Here’s an honest, step-by-step account of what to expect.

Most people who get in touch with me don’t quite know what to say.

That in itself tells me a lot. Not about how serious things are, or how long this has been going on, but about how much courage it took to get to the point of pressing send.

What follows is an honest, step-by-step account of exactly what the eating disorder counselling process looks like when you work with me, from your very first conversation through to ongoing therapy. No vague reassurances. No clinical jargon. Just the truth of what happens.

The fears that stop people reaching out (and why they're completely understandable)

I want to address the things people don’t always say out loud, because I hear them often enough to know they’re there.

"I'm scared you'll make me fat."

I won’t tell you what to eat. I won’t put you on a meal plan or set you a target weight. Eating disorder counselling isn’t about forcing you to eat differently. It’s about understanding the relationship you have with food, with your body, and with yourself, and working out together where that comes from and what might shift.

"I'm not sick enough to need this."

I hear this more than almost anything else. So let me be clear: you do not have to have reached a crisis point to deserve support. You don’t need a diagnosis. You don’t need to have been hospitalised or to be visibly unwell. If your relationship with food is causing you pain, that’s enough to reach out.

"I don't know what to say."

You don’t need to. That’s what the sessions are for. You can come in uncertain, not sure where to begin, not even sure how to describe what’s going on. Most people do. You won’t be expected to have it figured out before we start.

"I need someone to fix me."

This is the one I want to be really honest about. My Harry Potter wand is completely broken. Therapy is not something I do to you. It’s something we do together, and you’re the one who has to be willing to do the work. I will walk alongside you, every step of the way. But I can’t want your recovery more than you do.

That’s not a rejection. It’s actually the most hopeful thing I can tell you, because it means the change comes from you, and it’s yours to keep.

How the process actually works

Before I describe what happens in each stage, I want to give you the full picture. Working with me as a specialist eating disorder therapist isn’t the same as booking a standard counselling session. There are three stages, and each one matters.

First, a free 20-minute consultation. This is a no-pressure conversation where we find out if this is the right fit.

Second, a specialist 2-hour eating disorder assessment. Before weekly therapy begins, I carry out a thorough assessment that gives us a clear picture of where you are, how the eating disorder is affecting your life, and what we’re working towards.

Third, weekly 50-minute therapy sessions. Once the assessment is complete and we’re both happy to continue, we begin the ongoing work.

This process is more thorough than most people expect, and that’s intentional. It means that by the time we start weekly sessions, we’re not guessing. We know what we’re dealing with and we have a direction.

The first step is a free conversation, around twenty minutes, held online via Zoom.

This isn’t an assessment. It’s a chance for both of us to get a feel for whether this is the right fit. You can ask me anything you want to know about how I work. And I’ll ask you a little about what’s brought you here and what you’re hoping for.

There’s no pressure, no obligation, and nothing to prepare. You just need to show up and be honest.

The specialist 2-hour eating disorder assessment

This is where working with a specialist is genuinely different from seeing a general counsellor.

Before weekly therapy begins, I carry out a thorough two-hour eating disorder assessment. This gives me a proper understanding of you, not just the eating disorder, but everything around it.

We’ll explore your eating history and your relationship with food over time. We’ll talk about your current patterns, including exercise, routines, and how food and your body affect your daily life. We’ll look at body image, your relationships and support network, any relevant medical history, and what has or hasn’t helped in the past.

We’ll also talk about what you want things to look like going forward. If you imagine yourself as an eighty-year-old looking back, what do you want to have been able to do or feel that feels out of reach right now? That question tends to open something in people. It moves us away from the eating disorder as the whole story and towards you as a person with a life that matters.

By the end of the assessment, we’ll both have a clearer picture of where you are and what the work needs to focus on. We’ll also begin to explore what you think might have contributed to where you are today. There isn’t a right answer to that. It’s the beginning of a conversation, not a test.

What ongoing therapy sessions look like

Once the assessment is complete, we move into weekly online therapy sessions, each fifty minutes long via Zoom.

My sessions are informed by my training through the NCFED (National Centre for Eating Disorders), one of the UK’s most respected specialist eating disorder training bodies. That training gives our work a framework and a direction. But I don’t follow a script. I use it as one part of a wider toolbox, drawing on my master practitioner training and experience across other therapeutic areas, and I let what you bring to each session guide where we go.

In practice, that means I listen first. At the start of each session I’ll ask about your week. What went well? What felt hard? And here’s something I’ve come to believe strongly: the moments that didn’t go the way you wanted hold the most useful material. Your setbacks, your slips, your hardest days — that’s where the real strategies come from, and where the most meaningful progress gets made.

Between sessions, I ask most clients to use an app called Recovery Record, a clinician-supported tool designed specifically for eating disorder recovery. It allows you to log meals, moods, thoughts, and patterns between sessions, and I can monitor your entries through the clinician version. It gives me a window into your week rather than just the fifty minutes I see you, so I can understand what’s actually going on day to day and support you more effectively. You can also keep a paper diary if you prefer.

What I won't do

Just so there’s no uncertainty:

I won’t weigh you. There are no scales involved in our sessions.

I won’t prescribe a meal plan. That isn’t what eating disorder counselling does and it isn’t my role.

I won’t push you somewhere you’re not ready to go. The pace of the work is led by you.

I won’t pretend to have a magic answer. What I will do is be fully present, genuinely non-judgmental, and completely invested in your recovery for as long as we work together.

What happens if I'm concerned about your safety

Eating disorder counselling isn’t just talking. If at any point I’m concerned about your physical safety or wellbeing, we’ll discuss the next steps together. Sometimes that means working alongside your GP or specialist medical services so that you’re supported both psychologically and physically.

This is also why, before we begin working together, you’ll sign a working agreement and complete a lifestyle questionnaire. As part of that process, you’ll give your consent for me to contact your GP if that ever becomes necessary. This is a professional safeguard and a condition of working with me. If that doesn’t feel right for you, I’d rather know before we start than discover it’s an issue further down the line.

It also means that if your needs become more complex than I can safely support alone, I’ll be honest with you about it and help you navigate towards the right level of care. I don’t simply close the door.

When therapy might not be the right fit right now

I want to be honest about this, because I think it matters more than most therapists will say.

If someone else has pushed you to come, if a family member has booked you in or you’re arriving mainly to make someone else feel better, I’d gently suggest waiting. Therapy works when you’re the one who wants it. Not perfectly ready. Not fearless. Just genuinely willing to try.

If you expect me to do this for you, I won’t take your money. I mean that sincerely, and it applies no matter how long the waiting list is or how much I’d like to help you. It goes against my values as a therapist and as a person. And if the readiness isn’t there yet, the kindest thing I can do is tell you honestly and invite you to come back when it is. I will always welcome you back.

Frequently Asked Questions

How long does eating disorder therapy last?

There’s no fixed answer, because it depends on you, your history and what you’re working towards. I offer short, medium and long-term work. We’ll review together how things are going and what you need as we go, and you’ll never be kept in therapy longer than is helpful.

Do I need a GP referral to work with you?

No, you don’t need a referral to get in touch. However, as part of the working agreement you sign before we begin, and the lifestyle questionnaire you complete, you’ll give your consent for me to contact your GP if that ever becomes necessary. This is a professional safeguard and a condition of working with me. If that doesn’t feel right for you, I’d rather know now.

Do I have to recover completely?

Recovery looks different for everyone. Some people come to therapy wanting full recovery. Others want to find a more peaceful relationship with food and their body, to manage the harder days better, or to understand where the eating disorder came from. There isn’t one right destination. What matters is that we work towards something that feels meaningful for you.

What if I cry?

Then you cry. That’s what the session is for. There’s no need to hold it together or keep things professional. If tears come, they’re usually telling us something important, and we’ll follow them. You won’t be the first, and you certainly won’t be the last.

Do you work with young people as well as adults?

What if I'm not sure I actually have an eating disorder?

You don’t need a formal diagnosis to reach out. If your relationship with food is causing you distress, that’s reason enough to have a conversation. The specialist assessment will help us understand what you’re dealing with and whether I’m the right person to support you.
